Abstract Background The effects of an intensive prevention program (IPP) for 12 months following 3-week rehabilitation after myocardial infarction (MI) have been proven by the randomized IPP trial. The present study investigates if the effects of IPP persist one year after termination of the program and if a reintervention after >24 months (“prevention boost”) is effective. Methods In the IPP trial patients were recruited during hospitalization for acute MI and randomly assigned to IPP versus usual care (UC) one month after discharge (after 3-week rehabilitation). IPP was coordinated by non-physician prevention assistants and included intensive group education sessions, telephone calls, telemetric and clinical control of risk factors. Primary study endpoint was the IPP Prevention Score, a sum score evaluating six major risk factors. The score ranges from 0 to 15 points, with a score of 15 points indicating best risk factor control. In the present study the effects of IPP were investigated after 24 months – one year after termination of the program. Thereafter, patients of the IPP study arm with at least one insufficiently controlled risk factor were randomly assigned to a 2-months reintervention (“prevention boost”) vs. no reintervention. Results At long-term follow-up after 24 months, 129 patients of the IPP study arm were compared to 136 patients of the UC study arm. IPP was associated with a significantly better risk factor control compared to UC at 24 months (IPP Prevention Score 10.9±2.3 points in the IPP group vs. 9.4±2.3 points in the UC group, p<0.01). However, in the IPP group a decrease of risk factor control was observed at the 24-months visit compared to the 12-months visit at the end of the prevention program (IPP Prevention Score 10.9±2.3 points at 24 months vs. 11.6±2.2 points at 12 months, p<0.05, Figure 1). A 2-months reintervention (“prevention boost”) was effective to improve risk factor control during long-term course: IPP Prevention Score increased from 10.5±2.1 points to 10.7±1.9 points in the reintervention group, while it decreased from 10.5±2.1 points to 9.7±2.1 points in the group without reintervention (p<0.05 between the groups, Figure 1). Conclusions IPP was associated with a better risk factor control compared to UC during 24 months; however, a deterioration of risk factors after termination of IPP suggests that even a 12-months prevention program is not long enough. The effects of a short reintervention after >24 months (“prevention boost”) indicate the need for prevention concepts that are based on repetitive personal contacts during long-term course after coronary events. INTRODUCTION: The use of ozone, intra or periarticular, for knee and hip joint osteoarthritis (KO / HO) is clearly justified by its anti-inflammatory and antioxidants properties, that should diminish the arthritic episodes of this disease [1]. Several papers have proved the safety and efficacy of this treatment for KO [2-12], comparable to other classical treatments (steroids or hyaluronic acid (HA)) [13-14]. However, there is no paper about HO yet. STUDY DESIGN: This work is based on two open prospective studies started in February 2002 and stopped in February 2010, one group for each joint. Recruitment criteria for both groups were: - Kellgren& Lawrence KO classification: any - One/bilateral - No previous joint trauma - No rheumatic disorder - No previous surgery (but arthroscopic meniscectomy) - NSAIDs for at least two months - Promise to abandon any anti-inflammatory drugs during ozone treatment - Informed consent Clinical evaluation for KO was done using WOMAC questionnaire, pre-treatment and 1, 3, 6, 12 months after treatment. In case of HO, we used VAS for evaluation instead. CLINICAL DATA: All patients have, at least, 12 months follow-up. For KO, we have complied 199 patients (225 knees). There are two missing cases, not compiled, due to death at the 1 year revision. Age of sample ranged from 51 to 89 during the treatment. WOMAC pre-treatment was: -Pain 13.3 -Stiffness 5.6 -Function 46.2 Other data compiled, were: age, gender, BMI, Kellgren & Lawrence radiological scale (I to IV) and relapses (time free of symptoms). For HO, we have compiled 126 patients (133 hips). There are no missing cases at one year follow-up. Age ranged from 49 to 83. EVA pre-treatment was 7,33. Other data compiled, were: age, gender, BMI, use of imaging, Kellgren & Lawrence radiological scale (I to IV) and relapses (time free of symptoms). OZONE TECHNIQUE: All patients got one intarticular injection of ozone, once a week; in case of associated tendinitis or bursitis, a second or third injection was done together the intrarticular one. Injections were performed under strict asepsy. For the knee, we used a 27G x 30 mm needle with a syliconized syringe and a supero-lateral approach and. Ozone dose for intraticular injection was 15 mL at 20 mcg/mL.[15-16] Paratendon injection was performed with 5 mL at 20 mcg/mL. We always did a minimun of three intrarticular injections. Patients that did not improve were classified as failure. For the rest of the patients, the average number of injections was 4,8 (range 3 to 7). For the hip, we used a 25G x 90 mm needle with a syliconized syringe and a lateral approach. Ozone dose for intraticular injection was 5-10 mL at 20 mcg/mL.[15-16] Paratendon injection was performed with 5 mL at 20 mcg/mL. 45 patients were injected with imaging guide due to severe obesity. In these patients we used a 22G x 205 mm needle. We always did a minimun of three intrarticular injections. Patients that did not improve were classified as failure. For the rest of the patients, the average number of injections was 5 (range 3 to 10). RESULTS: From 225 knees, 44 (19.5% - the “bad result” group) did almost not improve at all; other rescue treatments were offered. The rest (80.5%) achieved a significant improvement, increasing WOMAC index over 25% of their basal level. The clinical improvement was obtained during the treatment or the first three months after treatment. WOMAC global improvement was 48%, including both groups. Relapses over the “good result” group have been of 8% at 1 year revision, and are statistically related just with Kellgren & Lawrence classification. We registered no side effect that needed further treatment. From 133 hips, 80% improved at least 2 points in VAS and 73% improved at least 3 points. The one month follow-up VAS score was 3,3 (improvement of 55%). From the patients that improved, 25% had a relapse at 1 year visit, and are statistically related just with Kellgren & Lawrence classification. The use of imaging support did not improved the results. We registered no side effect that needed further treatment. DISCUSSION: Results for KO are similar to Moretti's paper [12] and similar to the ones published for HA papers [15]. These last papers are almost always referred to 6 month follow-up. Comparing our results with HA papers at one year follow-up, they are clearly better. Longer term results for HA are even worse. This study has flaws due to its design, but similar design has been used for reporting results about drugs, HA or surgery, so comparison can be done. For HO, the results are even better that the one published for steroids or HA injections [17-18]. We agree with the publication about the use of imaging [19]. CONCLUSION: Ozone treatment in KO improves clinical outcomes over 25% of its base level in more than 80% of the patients. Relapse rate is 8% and is related with advanced osteoarthritis (Kellgren & LAwrence grades III-IV); minimal time free of symptoms is almost one year. The similarity with Moretti's results in a double blind clinical trial strength the indication for ozone in patients with KO. No paper has been published yet about HA, but comparing the results with steroids or HA injections, this treatment option is promising.

Abstract PurposeTo assess the safety and efficacy of prophylactic extraoral photobiomodulation (PBM) for the prevention of oral and oropharyngeal mucositis (OM) on clinical outcomes and survival in patients with oral cavity and oropharyngeal squamous cell carcinoma (OOPSCC).MethodsOOPSCC patients who received radiotherapy (RT) were prospectively randomized to two groups: prophylactic extraoral PBM and placebo. OM grade (NCI), pain (VAS), analgesia, and anti-inflammatory prescriptions were assessed weekly. Quality of life questionnaires (QoL) were performed at the first and last day of RT. Following RT, participants were evaluated quarterly for oncological outcomes follow-up.Results55 patients met the inclusion criteria. The first occurrence of OM was observed at week 1, for the placebo group (p = 0.014). Later OM onset and severity was observed for the PBM group, with first occurrence at week 2 (p = 0.009). No difference in severe OM incidence was observed (p > 0.05). Lower mean pain score was noted at week 7 for the PBM group (2.1) compared to placebo group (4.5) (p = 0.009). Less analgesics (week 3; p = 0.009/week 7; p = 0.02) and anti-inflammatory prescription (week 5; p = 0.0346) were observed for the PBM group. Better QoL scores were observed for the PBM group at last day of RT (p = 0.0034). No difference in overall survival among groups, was observed in one year of follow-up (p = 0.889).ConclusionProphylactic extraoral PBM can delay OM onset, reduce pain, as well as reduced analgesic and anti-inflammatory prescription requirements. Extraoral PBM was associated with better QoL. There was no evidence of PBM impact on oncological outcomes. TRN:RBR-4w4swx (date of registration: 01/20/2020)

Objectives: Meniscus tears are a common and significant source of knee dysfunction in active young adult patients, and no high-quality prospective cohort or RCTs studies exist evaluating patient-reported outcomes in patients in this age group with ligamentously stable knees. Our objective was to identify patient-reported outcomes and patient-specific risk factors from a prospective cohort with a minimum of one-year follow-up following meniscal repair or excision in patients with ligamentously stable knees. We hypothesized that both groups would have significant improvement in outcomes; patients undergoing meniscal repair would have a higher reoperation rate; and articular cartilage injuries, subsequent knee surgery, and certain demographic characteristics would be significant risk factors to inferior outcomes at one year. Methods: Between February 2015 and December 2017, ligamentously stable meniscal procedures were enrolled and prospectively followed using the outcomes management evaluation system (OME) at Cleveland Clinic. Patients aged 23-39 preoperatively completed a series of validated outcome measurements including the Knee Injury and Osteoarthritis Outcome Score for both Pain (KOOS Pain) and Quality of Life (KOOS QoL). At the time of surgery, physicians documented all intra-articular findings, treatment, and surgical techniques utilized. Patients were followed at minimum of 1-year postoperatively through the OME platform and asked to complete the same outcome instruments done at baseline as well as a question designed to evaluate the Patient Acceptable Symptom State (PASS). The incidence and details of any subsequent knee surgeries were also obtained. Multivariable regression analysis was used to identify significant predictors of outcomes. Results: A total of 371 patients aged 23-39 underwent meniscus excision or repair during the study period. One hundred ninety-four met inclusion criteria, and one-year follow-up was obtained on 72% (n = 139) of the cohort (67% male; median age 32). Both KOOS Pain and KOOS QoL improved significantly at one-year for the entire cohort. Fourteen percent of the cohort (9% on the ipsilateral knee, 5% on the contralateral knee) underwent subsequent surgery at a minimum of one-year postoperatively. The patient-specific risk factors for worse one-year outcomes included preoperative baseline mental capacity score (VR-12 MCS), lower baseline KOOS QoL score, and the intraoperative finding of any grade 3 or 4 chondral changes. Conclusion: Young adult patients with ligamentously stable knees undergoing meniscal surgery have significantly improved patient-reported outcomes regardless of excision or repair; however, 14% of patients underwent additional knee surgery at a minimum of one-year postoperatively. The risk factors for worse outcomes include lower baseline mental health score, lower baseline KOOS QoL score, and any grade 3 or 4 chondromalacia scene.

Objective: We analyzed the changes in voice- and swallowing-related symptoms that occurred over time in patients who underwent thyroidectomy and identified any associated risk factors. Methods: One hundred and three patients who underwent thyroidectomy were enrolled. Results: The mean thyroidectomy voice-related questionnaire (TVQ) score before surgery was 12.41 ± 12.19; it significantly increased to 28.24 ± 18.01 ( P < .001) 1 month postoperatively, decreased to 24.02 ± 17.30 ( P = .014) and 20.66 ± 15.29 ( P = .023) 3 and 6 months postoperatively, respectively. It was continuously decreased to 18.83 ± 14.63 twelve months postoperatively. The temporal changes in TVQ scores between patients who underwent total thyroidectomy or lobectomy were significantly different. There was a statistically significant difference in the temporal changes in TVQ according to whether neck dissection was performed. The temporal changes in TVQ in patients with and without extrathyroidal extension were significantly different. Conclusions: Voice- and swallowing-related discomfort in patients who received thyroidectomy showed dynamic changes over time. There was a significant difference in the degree of change according to clinicopathological factors. Patients with these risk factors may benefit from appropriate patient education and various rehabilitation programs for symptom relief.
